Family Medical Equipment - Client/Patient Bill Of Rights - Standard: DRX2-2A
As a patient of Family Medical Equipment (FME), you have the right to be treated with dignity and to be fully informed about your care. We believe that an informed patient is a safer patient.
Your Rights:
- Patient-Centered Care: To receive appropriate, high-quality care and equipment in accordance with your physician’s orders and your individual needs.
- Informed Consent: To be fully informed in advance about the care/services to be provided, including the frequency of visits and any modifications to your plan of care.
- Financial Transparency: To be informed, in advance of service, of your financial responsibility and any expected out-of-pocket costs.
- Dignity and Respect: To have your property and person treated with respect and recognition of your individuality and dignity.
- Staff Identification: To be able to identify all FME personnel through proper identification (Company ID badges).
- Safety and Protection: To be free from mistreatment, neglect, or verbal, mental, sexual, and physical abuse.
- Grievances: To voice complaints regarding treatment or lack of respect for property without fear of reprisal or discrimination. You have the right to a timely investigation of any grievance.
- Confidentiality: To have your clinical records and Protected Health Information (PHI) kept confidential and private in accordance with HIPAA regulations.
- Participation: To participate in the development and periodic revision of your plan of care.
- Refusal of Care: To refuse care or treatment after being fully informed of the medical consequences of that refusal.
- Advance Directives: To be informed of your rights under State law to formulate an Advance Directive (Living Will/Power of Attorney).
- Choice of Provider: To choose your own healthcare provider and attending physician.
Client/Patient Responsibilities
To ensure we can provide you with the best care possible, we ask that you fulfill the following responsibilities:
- Provide Information: Give accurate and complete health information relevant to your equipment needs.
- Follow Instructions: Follow the care plan and equipment instructions provided by FME staff and your physician.
- Safety: Maintain a safe environment in your home for the use of medical equipment.
- Notification: Notify FME immediately if your equipment malfunctions, if you are hospitalized, or if your insurance or contact information changes.
- Financial Responsibility: Fulfill your financial obligations for the equipment and services provided.
- Respect: Treat FME personnel with the same respect and consideration you expect to receive
